For GPs, referrers & Case Managers
We work closely with local GPs, allied health and Case Managers to look after your patients' and participants' feet and lower limbs — and to keep you in the loop. Referring is simple, and they'll be seen by a podiatrist who takes the time to get it right.

How to refer
- Book directly or call: your patient can book online, or phone us on (02) 9701 0477.
- Send a referral: email referrals to burwoodpodiatry18@gmail.com — include the reason for referral, relevant history/medications, and any imaging.
- Medicare care plans (GPCCMP): we welcome care‑plan referrals and process the rebate on the spot; your patient pays only the gap.
- DVA: we accept DVA Gold Card (and eligible White Card) referrals.
Funding schemes & Case Managers
We look after participants and clients across a range of funding schemes — and we understand what matters when you're coordinating care: approved budgets kept to, scheme admin handled directly, and reporting you can rely on.
NDIS — Plan & Self Managed
We're not a registered NDIS provider, but we can look after Plan Managed and Self Managed participants — coordinating with the plan and working within the approved budget.
Workers' Compensation & insurers
We work closely with Claims Advisors across SIRA, Workers' Compensation and insurance companies, and have extensive experience preparing the detailed reports approved cases need — with the insurer paperwork managed directly.
Support at Home
Podiatry delivered as part of a Support at Home plan, coordinated with your team.
External Aged Care schemes
We work with external Aged Care packages and their providers to keep care moving.

- Prompt appointments and straightforward coordination with your team — bring your Case Manager's details and we'll liaise directly
Workers' Comp & insurance approvals: let us know your Claims Advisor / Case Manager details and include the case number, and we'll coordinate directly. Prior written approval is required — please send it to burwoodpodiatry18@gmail.com before we proceed with assessment.
Support at Home & Aged Care providers: we're happy to review your service agreement — please forward any contracts to burwoodpodiatry18@gmail.com, and let us know which documents your team requires (for example AHPRA registration, professional indemnity insurance or a police check) so we can provide them.
Who'll see your patient
